To expedite the processing of your assignment, please ensure the following items are completed in full.
- Saskatchewan Crop Insurance Corporation contract number
- Your contract name and address
- Assignee's name and address. This name and address will appear on the cheque.
- Amount of assignment. If the amount is for the total indemnity, complete all the spaces with the digit "9".
- Indicate the crop year to which the assignment pertains. Note that all payments pertaining to that crop year will be assigned regardless of when the payment is made. The crop year is April 1 to March 31.
- Customer(s) and Witness(es) must print and sign name (must be original signatures). Customer(s) only will complete date.
- Complete and forward two original forms or the original and one copy to the corporation at the address noted on the assignment form. Faxed forms are acceptable. The corporation keeps the original and returns the second copy once processed.
- You will be able to receive information regarding your Assignment of Indemnity by completing the Authorization for the Release of Information form (PDF).
If possible, please type the form. Otherwise, print clearly. Assignment forms that are incomplete, incorrect or illegible will be returned.
An approved copy of the assignment will be returned to you, indicating the priority in which the form was received. However, prior to making any disbursements to an assignee, any offsetting amounts due by the customer in the federal-provincial program agreements will be deducted.
Should you require more copies of the assignment form, please feel free to make duplicates or print additional forms.
The assignment will cover all Crop Insurance benefits except establishment indemnities. New assignments are required for each crop year. Once the assignment is no longer required by the assignee, a written release is required.
