Application for Contract
If applying for or reinstating a contract of insurance, contact your customer service office.
Bin Location Map
This form is a document that is required for all new applications or reinstatements. All storage used by the applicant must be documented. Contact your customer service office or print the form, complete it and take it and required documentation into the customer service office.
Application Supplementary Form
If applying for or reinstating a contract, an Application Supplementary form must also be completed with supporting documentation attached. Contact your customer service office or print the form, complete it and take it and required documentation into your customer service office.
Confirmation of Verbal Land Rental Agreement
If you are renting land through a verbal land agreement, this form is required to confirm between the landlord and tenant that the agreement is verbal. It is required for all new applications and reinstatements and may be required for existing customers who take on new land in a verbal agreement. Contact your customer service office or print the form, complete it and take it and required documentation into the customer service office.
Land Change and Acres Seeded Form and Crops Identification Sheet
Used to report any land changes. On this form, you may add or delete land you operate.
Demographic Changes/Cancellation/TOEC form
If you want to cancel your contract of insurance or to transfer credits from an existing contract to another.
Power of Attorney Form
For a contract holder who wants to give power of attorney to another individual. The power of attorney can sign any Crop Insurance document except a contract application or cancellation.

Guarantee of Premium
When a corporation/limited company applies for a contract, Crop Insurance requires a guarantee from said individual(s) that the current year's premium will be paid.
Establishment Benefit Worksheet and Density Chart
You can approximately determine if your crop is established. If your calculations indicate your crop is not established, contact your customer service office to have an adjuster fully assess your field.
Seeded Acreage Reports and Instruction Guide
As a Crop Insurance customer, you are required to report your seeded acres, including crop variety and seeding dates. Depending on your operation, you are asked to complete one of two Seeded Acreage Reports before June 25.
2008 Stored Grain Declaration and Instruction Guide
All Crop Insurance customers are required to complete a Stored Grain Declaration form declaring any carry-over grain before June 25. Providing more detailed information allows Crop Insurance to more easily identify your stored grain, resulting in more efficient claim payments.
Pre-Harvest Worksheet and Data Sheet
If you wish to put your insured acres to a use other than harvesting, contact your customer service office before abandoning, baling, grazing, etc.
2008 Forage Declaration and form completion instructions
Your annual yield information is used to update your individual coverage. Even if you are not in a claim situation, forage declarations must be submitted by September 30.
2008 SMP/Organic Declaration and Reporting Your Production brochure
If you are a Saskatchewan Management Plus (SMP) or organic customer, your declaration form asks for additional information specific to your operation. The deadline to submit declarations is November 15.
2008 Production Declaration and Reporting Your Production brochure
Grain producers are required to submit their annual yield information to update their individual coverage. The deadline to submit declarations is November 15.
Request for Extension of Insurance
In the event harvest is not completed by November 15, you must request an Extension of Insurance. If approved, you are covered for over-winter losses.

Assignment of Indemnity and Instruction Sheet
With this form, the customer agrees to issue their cheques to a third party for a specified crop year.
Authorization for the Release of Information - Corporation, Individual, Partnership
All contract information is confidential and cannot be disclosed to anyone other than the customer and/or power of attorney. The customer must provide a written Release of Information form with a signature for information to be disclosed to a third party.
Authorization for Direct Payment
Authorizes Crop Insurance to deposit indemnities directly to the customer's bank account.

Endorsement Instructions and Maps

Grains Program
Your multi-peril insurance endorsement - select up to 80 per cent coverage on grains, oilseeds and pulse crops, with options to endorse the Contract Price Option and Crop Averaging Program.
Forage Insurance
Select yield-loss insurance on established perennial forages, sweetclover and greenfeed cereals. The Forage Establishment Benefit Option may be selected to cover losses during the forage establishment year.
Forage Rainfall Insurance Program
Protect your grazing acres against below normal precipitation amounts.
Annual Crop Weather Based Insurance Program
Endorsement for the basic $10 top-up, enhanced $25 top-up and $75 stand-alone insurance options, protecting against drought and early fall frost.
Corn Heat Unit Program
Protecting feed corn against lack of necessary heat units, this program is available in designated areas.
Wild Rice Insurance Pilot Program
The Wild Rice Pilot Program is an area-based insurance program based on average yields. Your coverage depends on the regions in which you harvest.
