This form is a document that is required for all new applications or reinstatements. All storage used by the applicant must be documented.

Confirmation of Verbal Land Rental Agreement (PDF File size: 25 K)

If land is rented through a verbal land agreement, this form is required to confirm that the tenant has access to the land under a verbal agreement. It is required for all new applications and reinstatements and may be required for existing customers who take on new land with a verbal rental agreement.

Insurable Crops Identification Sheet (PDF File size: 468 K)

This sheet provides the crop code for grain, forage and specialty crops insured by SCIC, as well as the codes of crops which can be insured under the Diversification Option or Forage Diversificatino Option.

Name/Demographic Changes/Cancellation/TOEC (PDF File size: 36 K)

To change contact information, cancel your contract or agree to the transfer of earned credits (TOEC), upon SCIC approval, please complete this form.

Land Change and Acres Seeded Form (PDF File size: 53 K)

This form is used to report any land changes on your contract. The form is also used to identify fall-seeded, perennial and biennial crops.

For a contract holder who wants to give power of attorney to another individual. The power of attorney can sign any document for their Crop Insurance contract, except a contract application or cancellation. View instructions on how to complete a Power of Attorney form.

Claims and Contract Maintenance

Application (PDF File size: 42 K)

Complete this form to apply for a Contract of Insurance. Applications must be made in person at an SCIC office prior to March 31.

Guarantee (PDF File size: 68 K)

When a corporation/limited company applies for a contract, SCIC requires a personal guarantee from said individual(s) that premium will be paid.

Seeded Acreage Report, General (PDF File size: 591 K)
Seeded Acreage Report, Irrigation (PDF File size: 566 K)

As a Crop Insurance customer, you are required to report your seeded acres, including crop variety and seeding dates. The deadline to report acres is June 25. To fill out the form, follow the instructions in the Seeded Acreage Report and Stored Grain Declaration Instruction Guide.

Stored Grain Declaration (PDF File size: 543 K)

All Crop Insurance customers are required to complete a Stored Grain Declaration form declaring any carry-over grain by June 25. Unreported carryover grain may be considered new grain in the event of a claim. To fill out your report on stored grain, follow the instructions in the Seeded Acreage Report and Stored Grain Declaration Instruction Guide.

Production Declaration: Forage (PDF File size: 402 K)
Production Declaration: Multi-peril and Organic (PDF File size: 340 K)

Production information serves two important functions; it is used to update annual yields and is required to register a post-harvest claim. If a Production Declaration is not completed, annual yield will be set at zero, reducing future coverage by 10 per cent for each year you fail to report. For help completing this form, follow the instructions in the Production and Forage Declaration Instruction Guide.

Request for Extension of Insurance (PDF File size: 372 K)

In the event harvest is not completed by November 15, producers must request an extension of insurance. If approved, overwinter losses are covered.

Fall-Seeded Acreage Report (PDF File size: 628 K)

This form is used to report all fall-seeded crops, including fall rye and winter wheat. The deadline to report acres is November 15. For instructions on how to complete this report, please see the following instructions.

Financial

Assignment of Indemnity (PDF File size: 470 K)

With this form, the customer agrees to issue their indemnity cheque to a third party for a specified amount and claim year. View instructions on how to complete this form.

Direct Depost Request (PDF File Size: 80 K)

This form authorizes SCIC to deposit claims and other compensation payments directly into the customer's bank account.

Grains Endorsement (PDF File size: 144 K)

The multi-peril insurance endorsement is used to change, add or delete crops, coverage levels, price options and program features. Changes must be made by March 31 each year, otherwise insurance remains the same as the previous year.

Forage Endorsement (PDF File size: 94 K)

The forage insurance endorsement is used for established perennial forages, sweetclover and greenfeed cereals. Coverage levels, price options and other program features are selected on this form.

Forage Rainfall Insurance Program Endorsement (PDF File size: 58 K)

This form is used to select insurance coverage for tame or native grazing acres. The insurance is based on precipitation at the selected weather station. Producers select forage type, weighting, weather station and acres for each legal land description each year by March 31.

Corn Heat Unit Program Endorsement (PDF File size: 54 K)

This form is used to select coverage for the Corn Heat Unit Program. Coverage level, weather stations and legal land descriptions must be provided each year by March 31.

Wild Rice Insurance Program Endorsement (PDF File size: 55 K)

As an area-based insurance program, wild rice producers indicate the region in which they are located, provide thei Saskatchewan Environment license number or band number, the number of acres in production and select their coverage level.

Honey Endorsement (PDF File size: 36 K)

This form is used to select coverage for the Honey Production Insurance Program. To select honey or make changes to existing coverage, this form must be submitted by March 31.
