This form can be used to request an amendment or submit additional information for your AgriStability program forms. Producers will be asked to provide additional information to justify the amendment. Producers can also use this form to provide SCIC with other additional information. Amendment requests should be submitted within 18 months of the receipt of your Calculation of Benefits (COB).

If you feel that program rules were not correctly applied in the processing of your application, you may request a review of the application by SCIC. Appeal requests must be submitted using the Appeal Submission form. All requests for appeals undergo an administrative review to determine if the request is considered an amendment by the Saskatchewan Crop Insurance Corporation’s (SCIC) guidelines. If you feel the nature of your request is an amendment, then please fill out the Additional Information and Amendment Request form.

The CWB-RA worksheet is an optional AgriStability program feature that you may use instead of the default calculation. The Worksheet allows you to calculate the CWB-RA using actual sales and the pricing option chosen when your CWB commodities were sold into the active pool. This worksheet is effective for producers who sell their CWB commodities using the initial price. The CWB RA worksheet ensures that CWB commodities sold at the initial price, and CWB commodities held in inventory, are valued at the same price. Please use the CWB RA Worksheet Guide when filling out the form. The instructions may have changed from previous years. It is important that you carefully review the information in the guide so your CWB-RA worksheet is filled out correctly ensuring your benefits are calculated accurately. Participants wishing to cancel participation in AgriStability must complete and return this form to SCIC by the applicable deadline. If you are changing to another type of farming entity (corporation, trust, etc) you must complete a Change Business Type form.

Farming operations that have gone through a change in their business structure in 2009 need to report that change as part of filing for AgriStability. For example, if a farming operation has transitioned from a sole proprietor to a partnership or corporation, simply complete the Change in Business Type Form. This form outlines the change and ensures your historical farm information is transferred to the new business structure.

Participants must notify SCIC if they would like to edit/add/delete a contact person. To do so, participants must complete and return this form to SCIC. In naming a contact person, you authorize that person to receive, provide or make changes to information on your behalf concerning the AgriStability program. When the Saskatchewan Crop Insurance Corporation (SCIC) requires additional information, they will contact the parties listed as your contact person(s) prior to the participant.

Participants who wish to change/update their personal information, such as name or address, can complete and return this form to SCIC. If you are changing your name you must supply supporting documentation such as a marriage certificate or other proof of your name change.

The Corporations/Cooperatives/Other Entities Form is used to report farming income and expense activities to SCIC. Trusts, Corporations and Co-operatives, Communal Organizations, Status Indians and Band Farms and Limited Liability Partnerships, that are participants in AgriStability, are required to submit financial and supplemental information to SCIC using this form. This information is used for both the AgriStability and AgriInvest programs and needs to be sent directly to SCIC. Please use the Corporations/Co-operatives/Other Entities Guide and Commodity Code Guide when filling out the form. The instructions may have changed from previous years. To ensure your application forms are filled out correctly and your benefits are calculated accurately, it is important that you carefully review the information in the guide.

The Harmonized AgriStability and AgriInvest Form for Individuals is to be used to report farming income and expense activities to the Canada Revenue Agency (CRA) and the administrators of the AgriStability / AgriInvest programs. The information you provide is collected by the CRA and although it’s used for tax purposes, through one easy step, you’ve also met one of the requirements to participate in AgriStability and/or AgriInvest. The CRA will share the AgriStability information with SCIC for the delivery of the AgriStability program. Please use the Harmonized Guide for Individuals when filling out the form. The instructions may have changes from previous years. It is important that you carefully review the information in the guide so your Harmonized AgriStability and AgriInvest Form for Individuals is filled out correctly ensuring your benefits are calculated accurately.

Note: The filing of these forms by themselves to the CRA may not constitute the filing of your 2009 income tax. Filing of income tax is still necessary to be eligible for AgriStability.

Participants who would like to apply for an interim benefit are required to complete and return this form to SCIC. The interim benefit option allows you to access funds prior to the completion of your fiscal period in the program year. The interim benefit is based on your estimated margin decline in the program year, relative your estimated margin decline in your reference period. To reduce the risk of overpayment, interim benefits are based on your estimated final benefit. Note: Because interim benefits are calculated from an estimate of a participant’s final benefit, overpayments can occur. An overpayment must be repaid by the participant.
New Participant Package
The New Participant Package includes the forms you will need to complete and return to in order to participate. The information collected on these forms is used to produce your enrolment/fee notice and to generate your participant fees. Forms must be completed and submitted to SCIC by the stated deadline. Participant Initial Declaration (PDF File size: 72.5 KB)
All AgriStability participants must sign the declaration to confirm they understand the terms set out by SCIC. The declaration outlines the roles and responsibilities of a participant in the AgriStability program. It is a one-time requirement that allows SCIC access to your AgriStability program information. Failure to submit this declaration may result in a loss or delay in your AgriStability benefits. The Participant Initial Declaration will be mailed to participants.
2009 Price Lists
This includes prices for commodities in the Commodity Code Guide. These prices will be used to value commodities reported in the crops/livestock inventory section of your AgriStability form. Crop and livestock inventories for market commodities are valued using both an opening price (P1) and a year-end-price (P2) and are referred to as the P1/P2 Hybrid Inventory Valuation method. With this method, changes in the value of your crop and/or livestock inventories will reflect changes over the course of the fiscal period and variations in the price of each commodity between the beginning and end of your fiscal period.
