Additional Information and Amendment Request Form
Participants can request an amendment to the information generated to calculate benefits. Producers will be asked to provide additional information to justify the amendment. Producers can also use this form to provide SCIC with other additional information. Available soon.
Appeal Submission Form
Participants who feel the guidelines were not accurately applied to their application can submit a request for appeal. For more information on the appeals process, please refer appeals.
Cancel Participation Form
Participants wishing to cancel participation in AgriStability must complete and return this form to SCIC by the applicable deadline.
Change Contact Person Form
Participants must notify SCIC if they would like to edit/add/delete a contact person. To do so, participants must complete and return this form to SCIC.
Change Participant Information Form
Participants who wish to change/update their personal information, such as name or address, can complete and return this form to SCIC.

Participants that file income under the following taxable entities are required to submit financial and supplemental information to SCIC:
- Trusts
- Corporations and Co-operatives
- Communal Organizations
- Status Indians and Band Farms
- Limited Liability Partnerships

Commodity Code Guide
CWB-RA Worksheet
This worksheet allows you to calculate the CWB-RA using actual sales and the pricing option used when your CWB commodities were sold into the active pool. This is an optional form. Available soon.
Harmonized AgriStability and AgriInvest Form for Individuals
Participants that file income tax under the following taxable entities are required to complete and submit this form to the CRA:
- Sole Propriertorships (including individuals in a partnership)
- Estates

Interim Application
Participants who would like to apply for an interim benefit are required to complete and return this form to SCIC. An Interim or Target Advance Payment (TAP) is calculated from an estimate of the participant’s final benefit. An interim benefit or TAP can result in an overpayment once the final benefit is calculated. An overpayment must be repaid by the participant. Available soon.
New Participant Package
The New Participant Package includes the forms you will need to complete and return to in order to participate. The information collected on these forms is used to produce your enrolment/fee notice and to generate your participant fees. Forms must be completed and submitted to SCIC by the stated deadline. Participant Initial Declaration
Participants are required to submit a signed declaration. The declaration outlines the roles and responsibilities of a participant in the AgriStability program. The participant initial declaration will be mailed to participants and will only need to signed and submitted to SCIC once.
